The LaTeX source for the document you requested will follow in three
separate mailings. Some time ago, I sent it to Oz to be included in the
Scheme repository, but for some reason it has not appeared there. Feel
free to use it however you like, so long as the authors' names and the
institution remain attached. Also, if you want to include an email
address, note that my address (below) has changed from the one to which
